Chronic Disease Management (CDM)

We’re delighted to be posting non Covid news! In 2020 the HSE and ICGP were just launching a new chronic disease programme when the pandemic struck. Although many of our older patients are already using this service in the practice, for obvious reasons it’s been a bit stop-start. Despite this, the early stages have been recognised by an international award.

We’re delighted to announce that in the coming months we’ll be rolling out this service to all eligible patients aged over 18. The practice will contact you when appointments are available. In the meantime, you can read more about this programme by using this link  Chronic Disease Management
